Pre-Approval Application

The most accurate and efficient way to initiate a pre-approval is to:
1) Click on the link below to download the application worksheet
2) Print it out
3) Complete the worksheet, and
4) Fax or mail it back to us.

You may also fill out an application online. Expect that this will take approximately 45 minutes and you will need financial and employment information on hand.

You will be asked to provide:
• A copy of your most recent paystub
• Copy of the past two year's W2 forms
• If self-employed or paid on commission, copy of past two year's Federal Tax Returns
• Copy of most recent asset account statement(s)
• If student in past two years, copy of your school transcript (student copy acceptable)
